When Might Root Canal Treatment Be Needed?

Possible signs include persistent toothache, pain when biting, prolonged sensitivity to hot or cold, swelling, gum tenderness, a recurring spot on the gum or darkening after trauma. However, some affected teeth produce few or no noticeable symptoms.

These symptoms can have several causes and do not automatically mean root canal treatment is required. A clinical examination and diagnostic tests are necessary.

Endodontic Examination and Diagnosis

The dentist examines the tooth, surrounding gums and bite. Testing may include responses to temperature, gentle pressure or tapping. Dental X-rays are commonly used to evaluate decay, existing restorations, roots and the bone surrounding the tooth.

The findings help determine whether the pulp may recover, requires treatment or whether the tooth has structural damage that limits restoration. Available alternatives, benefits, risks and expected prognosis should be discussed before treatment.

What Happens During Root Canal Treatment?

Local anaesthesia is normally used. The tooth is isolated, and an opening is created to access the internal canal system. Inflamed or infected tissue is removed, and the canals are cleaned, shaped and filled before the tooth is sealed.

Treatment may require one or more appointments depending on the tooth, infection, anatomy and symptoms. A temporary filling may be placed between visits.

Restoring the Tooth After Treatment

A root-treated tooth requires an appropriate final restoration to protect it and restore function. Depending on the remaining tooth structure and biting forces, this may involve a tooth-coloured filling, an onlay or a dental crown.

Avoid delaying the recommended final restoration because an inadequately protected tooth may fracture or become contaminated.

Root Canal Retreatment and Surgery

A previously treated tooth can occasionally develop persistent or recurrent symptoms. Possible options may include nonsurgical retreatment, root-end surgery or tooth removal, depending on the cause and restorability of the tooth.

Retreatment may require removal of an existing crown, filling, post or root-filling material. Root-end surgery may be considered when inflammation or infection persists around the end of a root and conventional treatment cannot adequately address it.

Benefits, Limitations and Risks

Root canal treatment aims to retain a suitable natural tooth, but success cannot be guaranteed. Possible complications include persistent infection, instrument separation, perforation, untreated anatomy, tooth fracture and the need for retreatment, surgery or extraction.

Antibiotics are not a substitute for dental treatment in most localised tooth-pulp conditions. They may be prescribed when the clinician identifies signs that justify their use.
