What Is an Apicoectomy?
An apicoectomy is a surgical procedure that treats the area around the end of a tooth root. It may be considered when inflammation or infection persists after root canal treatment and cannot be adequately managed through conventional retreatment.
Assessment and Alternatives
The dentist evaluates the previous root canal, restoration, tooth structure and surrounding bone. Dental X-rays or three-dimensional imaging may be required.
Possible alternatives include nonsurgical retreatment, monitoring in selected circumstances or extraction. Surgery is considered only when the tooth has a reasonable restorative and periodontal prognosis.
How Is Root-End Surgery Performed?
After local anaesthesia, a small opening is made through the gum near the root. Inflamed tissue and a small part of the root tip are removed. The end of the canal may be prepared and sealed before the gum is repositioned with sutures.
Recovery and Risks
Temporary swelling, bruising or tenderness may occur. Possible risks include infection, bleeding, altered sensation, sinus involvement, gum recession, damage to nearby structures and failure to resolve the condition.
Follow instructions concerning cleaning, food, activity and prescribed medicines. Attend the scheduled review and suture-removal appointment.
