What Are Dental Inlays and Onlays?

Dental inlays and onlays are custom-made restorations used to repair suitable teeth affected by decay, fractures or large existing fillings. They may be considered when a standard filling cannot provide sufficient support but the tooth does not require full coverage with a dental crown.

An inlay fits within the raised edges, known as cusps, of the tooth. An onlay covers a larger area and may extend over one or more cusps. The restoration is shaped to fit the prepared tooth and restore its biting surface.

When May an Inlay or Onlay Be Recommended?

Your dentist may consider this treatment when a back tooth has moderate decay, a damaged filling, a fracture or weakened areas that require additional support. Whether it is appropriate depends on the remaining healthy tooth structure, the location of the damage, biting forces and the condition of the tooth and gums.

Smaller areas of decay may be treated with tooth-coloured fillings. More extensively damaged teeth may require a crown or another treatment. A clinical examination and, when necessary, dental X-rays are required before making a recommendation.

Materials Used for Inlays and Onlays

Inlays and onlays can be made from ceramic, composite resin or other dental materials. Ceramic restorations are often selected when strength, wear resistance and a tooth-like appearance are important. Material selection is based on the tooth’s position, the amount of remaining structure, the bite and the patient’s individual needs.

What Happens During Treatment?

1. Assessment: The dentist examines the tooth and may take an X-ray to determine the extent of the damage.
2. Tooth preparation: Decay or an unsuitable existing restoration is removed while preserving healthy structure where possible.
3. Digital scan or impression: A record of the prepared tooth is taken so the restoration can be designed.
4. Restoration placement: The inlay or onlay is checked for fit, appearance and bite before being bonded to the tooth.
5. Final checks: The bite is adjusted when needed, and cleaning instructions are provided.

Some cases can be completed in one appointment, while laboratory-made restorations may require two visits and a temporary restoration between appointments. The exact process depends on the selected material and clinical situation.

Aftercare and Maintenance

Mild sensitivity may occur temporarily after treatment. Avoid chewing until any numbness has completely worn off. Brush twice daily, clean between the teeth and attend regular dental examinations. Avoid biting very hard objects, and contact the clinic if the restoration feels high when biting, becomes loose or causes persistent discomfort.

The lifespan of an inlay or onlay varies according to its material, position, biting forces, oral hygiene and habits such as teeth grinding. A custom night guard may be recommended when grinding or clenching could place the restoration under excessive pressure.
